In order to train with us, we need to collect certain types of information from you. We would like you to be clear on the personal information we collect and why.
The information we collect and why and how we do it
Through our physical activity readiness questionnaire and initial assessment, we collect and process (that is use, transfer and store) the following information from you:
- Your name, email address and phone number. This is so we can contact you with regards to training or invoicing.
- We ask you to provide us with your home address and details of your emergency contact details in the unlikely event that we need to alert anyone about your health during a session.
- Your date of birth, high-level medical information and pregnancy status so our coaches can be aware if you are working with any health issues. You can of course choose not to give us this information but you must accept that this may limit our ability to coach you. We also ask you to consult your GP if you are unsure if you should be exercising.
We will not give your information to a third party without your permission.
How we store your personal information
Your information is securely stored on online servers. We keep your personal contact and medical details only for as long as is necessary; we will review this need from time to time. Of course, if you ask us to delete this information from our records sooner, we will do so.
You may ask to see copies of the information we hold on you at any time. Being a small business, it may take us up to 30 days to complete this task but we will keep you informed as we go.
If you need to contact us
Please email us at info@leahdunthorne.com and we will get back to you as soon as we can.
How to complain
We really hope you do not need to, but should you have any concerns about our use of your personal information, you can make a complaint to us at info@leahdunthorne.com
You can also complain to the ICO if you are unhappy with how we have used your data.
